Horizon knows there are many challenges posed from COVID-19. Work, education, childcare and safety being a few that have disrupted our routine these past few weeks. However, taking care of yourself should not be one of them. We cannot emphasize it enough. You have to take care of yourself. If you are not well, whether physically or mentally, you cannot take care of others. That is why Horizon continues to offer support and treatment as safely as possible during this COVID-19 period. Mental Health Services

Substance Use Services
Substance use is complicated. We understand that. It may have impaired relationships with your family or friends, or negatively impacted your health or career. Not everyone’s challenges are the same. This is why we continue to develop programming and services to meet your unique needs.

- Family support & Parent and Family Support Coordinator
- Medication assisted treatment (MAT) assessments
- Psychiatric medication management
- Codependency services

- Recovery Coaching: Recovery Coaching is a form of strength-based support for Horizon patients in or seeking recovery from alcohol and/or other drugs. It is a partnership where the person in or seeking recovery self-directs their recovery while the coach provides expertise in supporting successful change. Recovery coaches support any positive change, helping persons coming home from treatment to avoid a reoccurrence, build community support for recovery, or work on other life goals such as relationships, work, education etc.
Care Coordination Program
Our Care Coordination program works with patients to help them manage their complex and chronic health care needs. Care Coordinators provide outreach, engagement and care management services to eligible persons enrolled in New York’s Medicaid program.
