Horizon’s Stronger Together Action Plan guides our efforts in advancing health equity within our organization, community, and for those we serve. Our plan includes three commitments that drive actionable change in moving us forward with our goals.

We will enhance and expand our community relationships through active engagement with community members and organizations. Through community events and education, we promote inclusion and address barriers to access. Through our community partnerships and collaborations, we strive to reduce health disparities and expand opportunities for training and social impact.
Join Us for a Community Networking Event!
As we continue our ongoing efforts and commitment to Diversity, Equity, Inclusion, and Belonging (DEIB), Horizon is hosting a Stronger Together Community Networking Event! We are excited to improve collaboration and engagement efforts for individuals across organizations throughout Western New York. The goal of this event is to build and strengthen our community network and in turn enhance the support and service availability for patients and community members.
