Frequently Asked Questions (FAQs)
We strive to have counselors available for assessments the same day you call. Based on call volumes and your availability, we most often can schedule you for an appointment at one of our outpatient locations within 1-5 days. Please note that availability at each outpatient location varies, which may affect which location we can schedule you at for an initial appointment. Evaluation appointments are also available virtually on our telehealth platform. We have staff available that can help you connect.
Horizon offers in-person open access for mental health and substance use evaluations. Patients are seen on a first come, first served basis. Your first appointment will last approximately one hour. During that appointment, a counselor will spend time getting to know you, your history and symptoms, and your goals for treatment. Open access appointments are available for adults ages 18+. Click here to learn more about our Open Access locations. Insurance cards are required at initial appointment. Please call our Patient Engagement Center Team at 716-831-1800 to learn more and for appointment availability.
We are committed to providing the highest quality and most cost-effective care. We accept most commercial health insurance plans, Medicaid, and Medicare. Please note that insurances accepted vary based on service type provided, such as outpatient treatment vs. inpatient and residential treatment. Your financial responsibility is determined by your individual contract with your insurance company. Our staff will assist you in determining that amount. If you do not have insurance that covers our care, you may qualify for a reduced fee based on your income. Whatever your financial status, our team is here to assist you with understanding your unique situation and to access treatment.
We currently accept the following:
- Aetna/Meritain (not in network with Aetna Medicare)

If insurance is not listed IN NETWORK above, it is more than likely out of network and would be full fee for all services.
If you do not see your insurance plan, please contact us at (716) 831-1800 and a Financial Resource Specialist from our team will be happy to check your coverage. You may also contact your insurance provider for additional information.
We can assist you in applying for Medicaid. If it appears that you may be eligible for Medicaid, we will connect you with a Financial Resource Specialist who will help you begin the application process. You also may qualify for a reduced fee, and we will discuss those options with you as well.
