A Certified Community Behavioral Health Clinic (CCBHC) is a clinic that provides comprehensive, integrated mental health and substance use disorder services. Our CCBHC programs offer a wide range of services to patients of all ages with a focus on quality evidence based practices, improving overall health outcomes and increasing access to services to our community.

Horizon’s Lockport Recovery Center and Pine Avenue Recovery Center in Niagara County are both designated CBCHCs.

Our counselors provide compassionate, evidence-based care for individuals facing mental health and substance use challenges. They are trained to help you manage symptoms of depression, anxiety, trauma, suicidal thoughts, perinatal and postpartum symptoms. In addition, they offer support for substance use concerns, including harm reduction strategies and access to helpful resources.

Our skilled Medical Team is here to support your mental health and wellness with expert, compassionate care. We bring together professionals trained in psychiatry and addiction medicine to help you on your path to recovery.

  • Psychiatric Mental Health Assessment and Care – Our team of specialists provide psychiatric evaluations, offer education about your symptoms, and can prescribe medications when needed to support your treatment plan.
  • Help for Substance Use – We offer support for drug or alcohol use, including Medication-Assisted Treatment (MAT) when appropriate. Our Addiction Medicine specialists work with you to find the best path to recovery.

Our experienced Registered Nurses provide medical assessments, education on health conditions such as diabetes, high blood pressure, and asthma. They can complete blood draws onsite and also administer long-acting injectable medications prescribed by Horizon Health providers. Nurses are available to answer questions about medications, health issues, and to coordinate care with your medical provider.

  • Administrative Assistants: Administrative Assistants are your first contact and can connect you to your counselor or relay a message from you to your counselor. They are also available to assist you with updating contact information, scheduling and rescheduling appointments, telehealth troubleshooting, transportation scheduling, records requests and directing medication questions and refills to the appropriate parties.
  • Medical Operations Specialists: Medical Operations Specialists are available to help you communicate with your provider, they can assist with medication questions and refills, facilitating referrals your provider recommends, and medication prior authorizations from your insurance carrier.
  • Clinical Care Specialists: Our Clinical Care Specialists work closely with your counselor to assist in your treatment. They may assist you in times when your Counselor is not available.
  • Peer Advocates: A Peer Advocate is a helpful support in the road to recovery. By offering peer support, a Peer Advocate can meet you in the community and help remove barriers to your care. Peer Advocates are individuals with lived experience who share their experience, strength, and hope to help patients reach their goals.
  • Parent & Family Support Coordinators: Our Parent & Family Support Coordinators offer support to family members of loved ones in our care to help them understand the road to recovery and support their loved one.
  • Care Coordinators: Our Care Coordination Program assists people with serious or long-term health problems get the care they need. A care coordinator works with each person to create a plan that connects them to doctors, mental health support, housing assistance, and other services. The goal is to keep people healthier and out of the hospital by making sure they get the right care at the right time.
  • Targeted Case Managers: Targeted Case Managers play a vital role in supporting individuals throughout their recovery journey. They assist with transitions of care from higher levels of care to outpatient settings, ensuring continuity and stability. Targeted Case Managers provide linkages and referrals to medical providers and essential community resources, including food, clothing, housing, and transportation. They also support individuals with medication adherence by reinforcing treatment plans and coordinating with prescribers as needed. In addition, they offer general support and guidance to empower individuals in achieving their personal and health-related goals.
  • Psychiatric Rehabilitation Specialists: Psychiatric Rehabilitation Specialists aim to help you develop the skills and support systems necessary to do things like managing routines, accessing education, preparing for work, and leading a fulfilling life.
