Make an appointment

What to expect from your first appointment

You will meet with the Registrar, who will:

  • request some basic information (address, date of birth, phone number, Social Security number, etc.);
  • make a copy of your health insurance card and referral form, if applicable;
  • inform you regarding your health insurance coverage (if known);
  • ask you to sign authorizations to receive your health information from your past and current treatment providers and information from the person or agency that referred you to us (if applicable); and
  • possibly have you meet with a Resource Advocate to determine whether you may be eligible for Medicaid (and to arrange to assist you in filing an application).

The next person you meet will be a Counselor, who will:

  • speak with you regarding your situation, needs, goals and expectations; and
  • use this information to work with you to develop an individualized recovery and rehabilitation plan that includes selected activities and a physical health screening.
