Paying for Services

Horizon accepts Medicaid and Medicare, as well as most area managed and non-managed health insurances. When you call for an appointment, our staff may be able to advise you whether your treatment may be covered in total or in part by your health insurance. However, we encourage you to call your insurance carrier to clarify the nature and extent of your coverage. At your first visit, a written fee agreement will be established and our payment policies will be discussed in detail.  

Fee Reductions: You may be eligible for a fee reduction based on your income if you:

• meet federal poverty guidelines; or
• do not have health insurance

Documentation of your income (e.g., W-2, last three pay stubs, tax form) is required to be eligible for a reduced fee. If you plan to apply for a reduced fee, you must bring this documentation with you to your first visit. We will also help you apply for Medicaid or other government entitlement programs for which you may qualify.

Vocational Program services are not covered by health insurance. These services are paid for through private and government grants and government agencies that fund vocational services for persons with disabilities.
