Scheduling an Initial Appointment

To schedule an initial appointment at any Horizon Health Services location call (716) 831-1800 between the hours of 9:00 a.m. and 5:00 p.m., Monday through Friday and ask to be connected to our Central Intake Department. 

A member of our staff will take some identifying information and ask you about the types of services you are seeking. The staff member will then schedule an appointment for you at a location and time that are convenient. This staff member will also be able to describe the range of services Horizon provides, and answer your insurance and other payment questions.

If you have health insurance, you will need to bring: If you do not have health insurance, you will need to bring:

• Your insurance card

• Cash, check or credit card to pay your insurance co-pay

• Photo Identification

• Birth Certificate

• Your Social Security Card

• Proof of Address (current utility bill or postmarked mail)

• Your last 3 pay stubs (if applicable)

• Cash, check or credit card to pay for your first appointment.
