Horizon Village Admission Application
Applicants for admission to Horizon Village must meet all of the eligibility criteria described below in order to be considered for admission. Referral sources are expected to ensure that individuals they are referring meet all of the criteria prior to submitting an application.
Referral
sources must ensure that the applicant:
- Is at least 18 years of age.
- Presents no indication of acute withdrawal symptoms.
- Presents at least low average intelligence and no significant cognitive impairments.
- Presents a demonstrated inability to maintain abstinence from all mood-altering substances despite previous inpatient or outpatient treatment.
- Presents substantial deficits in functional skills evidencing the need for extensive habilitation or rehabilitation.
- Presents written substantiation of a chemical dependence diagnosis consistent with DSM-IV.
- Presents medical and psychiatric stability, evidenced by the applicant;
- Appearing to be free of serious communicable diseases that can be spread through ordinary contact, and
- Appearing to not be in need of acute psychiatric treatment or medical hospitalization.
- Is expressing agreement and commitment to participate in intensive residential treatment.
