Horizon Health Services is a not-for-profit organization that provides an array of comprehensive services to assist individuals with mental health problems and/or addictions to make choices that improve their quality of life and achieve their personal recovery goals.

We empower consumers and support their pursuit of their hopes and dreams through an integrated service delivery system. Consumers are partners in selecting and implementing their care from among high-quality services that are accessible and responsive to their needs and desires.

Horizon's services are delivered out of 10 facilities located in Erie, Niagara and Genesee Counties, with a concentration of services in the cities of Buffalo and Niagara Falls. The various clinical programs are co-located within facilities to ensure the integration of all needed care for persons with multiple disorders. Service plans are based on individualized needs and choices rather than fixed program designs.

Horizon Health Services is unique in that it:

• provides fully integrated behavioral health, medical and vocational rehabilitation services in community-based settings;
• is committed to progressive, innovative and evidence-based practices;
• provides a broad range of comprehensive and specialized services;
• provides fully integrated vocational assessment, placement and job support services; and
• maintains provider contracts with all major area health insurance companies.

A volunteer board of directors comprised of community members performs governance functions. Horizon Health Services accesses executive management services from its corporate affiliate, Health Management Group, Ltd.

All clinical services are provided under the auspices of the regulatory body with appropriate jurisdiction (NYS Office of Alcoholism and Substance Abuse Services, Office of Mental Health, Department of Health).
